Mitchell's Pharmacy is committed to protecting the privacy of your health information. We are required by law to maintain the privacy of your protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

We may use and disclose your health information for treatment, payment, and health care operations. For example:
- Treatment. We may use your information to fill prescriptions and to coordinate your care with your physicians and other providers.

- Request access to and a copy of your health records.

- Request restrictions on certain uses and disclosures.
- Request confidential communications.
- Receive a list of certain disclosures we have made.
- Receive a paper copy of this notice upon request.
- File a complaint if you believe your privacy rights have been violated, without fear of retaliation.
Our responsibilities
We are required by law to maintain the privacy and security of your protected health information, to let you know promptly if a breach occurs that may have compromised the privacy or security of your information, and to follow the duties and privacy practices described in this notice. We will not use or share your information other than as described here unless you tell us we may in writing.
Your right to file a complaint
If you believe your privacy rights have been violated, you may file a complaint with Mitchell's Pharmacy or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

- With the federal government: File a complaint with the HHS Office for Civil Rights online at hhs.gov/ocr/privacy/hipaa/complaints, or by mail to the U.S. Department of Health and Human Services, Office for Civil Rights, 200 Independence Avenue SW, Washington, D.C. 20201.
Changes to this notice
We may change this notice and the changes will apply to all information we hold about you. The current notice is always posted on this page with its effective date shown above, and a paper copy is available at either of our locations upon request.
